Data controller
The data controller is PHYSIOWORKS — Physiotherapy Center, Pergamou 5, Nea Smyrni 171 21, Attica, Greece. For any matter relating to your data you can contact us at physioworks5@gmail.com or on 211 423 9253.
What data we collect from the website
The website is static and has no database or user registration system. We collect data only in the following cases:
- Contact form: your name, phone, email and the text of your message. The form does not store data on a server — it opens your device's email application with the message pre-filled, which is then sent to physioworks5@gmail.com.
- Contact by phone, WhatsApp or email: the details you choose to provide during that contact.
- Technical server data: our hosting provider logs IP address, access time and browser type for security and service stability purposes.
Sensitive health data
Please do not send detailed health information, diagnoses or medical documents through the contact form or by email. To describe your case, call us or discuss it at your first visit.
Health data arising during your treatment at the centre is held in a separate clinical record, outside the website, in accordance with medical confidentiality law and our obligations as a healthcare provider.
Legal basis and purpose of processing
We process the data you send through the form solely to respond to your enquiry and arrange an appointment. The legal basis is your consent and the taking of steps at your request prior to entering into a service agreement (Article 6(1)(a) and (b) GDPR).
Cookies and third-party tools
The website does not set tracking or advertising cookies. Two third-party services are used, however, and may receive your IP address:
- Google Fonts — to load the page typefaces.
- Google Maps — for the embedded map in the contact section. The map loads only when you scroll to that part of the page.
Retention period
Contact messages are kept for as long as needed to handle your enquiry and, if you become a patient, for as long as healthcare provider record-keeping law requires. Messages from people who did not proceed to an appointment are deleted once they are no longer necessary.
Your rights
Under the GDPR you have the right to access your data, and to rectification, erasure, restriction of processing, portability and objection, as well as the right to withdraw consent at any time. To exercise any of these, contact physioworks5@gmail.com.
Lodging a complaint
If you believe the processing of your data breaches the law, you have the right to lodge a complaint with the Hellenic Data Protection Authority (www.dpa.gr).
